When you file for bankruptcy, federal law requires you to disclose information about, among other things, all of your debts, assets, income and expenses. Failure to do so is a federal felony. In order to facilitate the face-to-face consultation with you, we created the following forms. While these are not official forms, all the information requested is necessary for us to give you legal advice and prepare your bankruptcy paperwork. If you can take time to fill them out in advance and bring them to the interview, it will significantly expedite the interview process and save time for both of us. If there are questions you don’t understand or if you are not sure about the answers, don’t worry—we will go over those with you. We greatly appreciate your assistance.
